What to Do If You Have a Complaint
All complaints or concerns other than discrimination or sexual harassment allegation must be brought to the immediate attention of the site supervisor, case manager, or instructor within 10 working days of the triggering incident. In the case where the grievant is not satisfied with the resolution, then a written complaint may be filed by completing a Complaint Form, PF-208C within five (5) days. Send the Complaint Form to the Proteus, Inc., Jeana Brooks Affirmative Action/Equal Employment Opportunity Officer (AA/EEO) at the Human Relations Department 1900 N. Dinuba Blvd., Visalia, Ca 93291, (888) 776-9998. The AA/EEO Officer will log the complaint and forward to the appropriate Division Director who will attempt to resolve the matter within five (5) working days. The complaint procedure provides for a review, investigation and presentation of findings regarding the alleged complaints by the Affirmative Action /Equal Employment Opportunity Officer (AA/EEO) – Jeana Brooks located at 1900 N. Dinuba Blvd., Visalia, Ca 93291; an informal hearing and determination by the Chief Executive Officer as necessary; and if required, a formal hearing and determination by an impartial hearing officer.
